Sep 29, 2026 - 11:58 EDT Investigating - We are investigating reports that some Formstack Sign customers are unable to send documents through the API. Affected requests may return a “Request requires authentication” error despite successfully generating an access token. Some customers may also see a null API secret within their account. Our team is actively investigating and will provide additional updates as more information becomes available. Sep 29, 02:00 EDT Update - The Sign migration is nearly complete and we are in the testing phase. Users of Sign should see normal operation. All documents in flight continued to get signatures throughout the migration. There was a short period of time where users would not have been able to log into Formstack Sign from about 12:15 to 12:30am Tuesday morning 9/29. Sep 29, 01:11 EDT Monitoring - This migration work has begun and is progressing smoothly. Sep 28, 22:07 EDT Identified - Future: At approximately 9:30 pm Eastern Time on Monday September 28th, Formstack Sign will undergo a migration to ensure faster and more reliable user authentication. There is no risk to documents in flight, and no risk of account data loss. There could be a small interruption to user sessions during the migration, which is expected to take nearly two hours. Sep 28, 16:01 EDT Documents with Sign Delivery are failing Resolved - Incident resolved. Documents deliveries to Formstack Sign were failing from approximately 11:15 pm Eastern time, and successful deliveries were restored at approximately 1:35 am Eastern time. Many deliveries will auto-retry. However, customers should review their deliveries in the Formstack Documents interface to ensure no action is required. Sep 29, 01:59 EDT Monitoring - A fix has been deployed and deliveries should be back up and running. We will monitor the results for some time and report back. Sep 17, 15:15 EDT Monitoring - A fix has been implemented for the issue causing some users to receive a validation error when entering a phone number in the Phone Number field on Forms. We are monitoring to ensure the issue remains resolved. Our team has identified the cause and is working to resolve the issue. Sep 17, 10:48 EDT Sep 16, 2026 Salesforce Service Disruption Resolved - Salesforce has resolved the service disruption, and affected services have returned to normal operation. We have reviewed the impact of the outage across our Salesforce-connected products. Recovery requirements vary by product: Forms: Customers with failed Salesforce integrations will have received a failed integration notification. Affected submissions can be selected and the integration re-run from within Forms. Forms for Salesforce Classic: We will broadly resubmit failed submissions impacted by the outage. No customer action is required at this time. Forms for Salesforce Native: Because affected Salesforce orgs were unavailable, forms could not be served during the disruption. There are no submissions to recover; any forms that were not completed will need to be submitted again. Documents for Salesforce: Merge requests could not be initiated while affected Salesforce orgs were unavailable. Any required merges will need to be triggered again. Formstack Documents — Salesforce Deliveries: Depending on the configuration, failed merges or deliveries may need to be re-triggered. Please contact Support if assistance is needed. Sep 16, 08:49 EDT Identified - Salesforce is currently experiencing a service disruption impacting multiple regions. This may result in delays, intermittent errors, failed submissions, or difficulty accessing Salesforce-connected services. Salesforce has deployed a fix and services are recovering region by region. We are actively monitoring the recovery and assessing any impact to Intellistack customers. We will provide additional updates as more information becomes available.
