EZAppeal Legal Terms of Service These terms are the Underlying Agreement referenced by the EZAppeal Business Associate Agreement. EZAppeal Terms of Service Last Updated: September 22, 2026 1. Service Description EZAppeal provides AI-powered tools to assist healthcare providers in generating insurance appeal letters and prior authorization requests. Our service analyzes clinical documentation and generates professionally formatted letters based on payer-specific criteria. 2. Data Processing & Privacy Important: To provide our AI-powered services, clinical documentation you input is processed by our AI system. Please note: No Permanent Storage: Clinical notes, patient information, and generated letters are NOT permanently stored on our servers. Data is processed in real-time and discarded after your session. AI Processing: Your data is processed through AWS Bedrock, with which we maintain a Business Associate Agreement (BAA) for HIPAA compliance. AWS Bedrock does not use your data for training and provides enterprise-grade security. Data is processed in real-time and not retained. What We Keep: We keep records tied to your account: your account and practice details, saved provider letterheads, a short activity record for each document drafted (payer, procedure and denial codes, billed amount, criteria-match counts, document type and time, and any sent date or payer decision you record), a usage record (events and counts), sign-in history, and payment records. None of these contain patient names, member IDs, dates of service, or document text. Our Privacy Policy describes what we keep and how long we keep it. Your Responsibility: You are responsible for downloading and saving any generated documents before leaving the page. We cannot recover documents after your session ends. 3. HIPAA Compliance By using EZAppeal, you acknowledge that: You are authorized to input patient information for the purpose of generating appeals You will maintain appropriate safeguards for any downloaded documents containing PHI EZAppeal maintains a BAA with AWS for HIPAA-compliant AI processing 4. Disclaimer EZAppeal uses artificial intelligence to generate appeal letters and prior authorization requests. AI-generated content can contain errors, inaccuracies, or outdated information. All generated documents are suggestions only and must be reviewed, verified, and approved by a qualified healthcare professional before submission. You are solely responsible for the accuracy and completeness of any documents you submit. We do not guarantee appeal approval or claim outcomes. 5. Limitation of Liability EZAppeal is provided "as is" without warranties. We are not liable for any damages arising from use of our service, including but not limited to denied appeals or claim outcomes. 6.
